How ShelterCare Works
Activation Methods
Optional - Adopter is given ShelterCare materials and allowed to activate on their own by calling ShelterCare at 866.375.7387 or through www.sheltercare.com
Mandatory - Shelters using the
24PetWatch microchip program can activate the ShelterCare gift on
behalf of the adopter after informing the adopter. The shelter must
activate the gift using PetPoint or the EPE utility only! Paper forms not accepted.
Activation Period
The ShelterCare gift must be activated within 10 days of the adoption date.
Coverages
Covers the 11 most common illnesses and accidents:

| Pre-existing conditions are not covered under the ShelterCare program |
Coverage Period / Coverage Amount
Coverage begins 1 second past the second midnight from the date of activation. Coverage continues for 30 consecutive days unless adopter chooses to extend coverage or move to an annual policy.
Claims
Claim form must be completed and signed by the attending DVM.

Claims must be submitted within 90 days of the policy expiration date
Claims are paid after the applicable deductible of $50/incident
